Duties

• Effectively screen and evaluate patients with physical functioning disorders and/or wound care and develop appropriate plans of care following all regulatory and clinical practice standards.

• Provide rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care per regulatory and clinical practice requirements.

• Administer various procedures as part of the rehabilitation plan, including manual techniques; ambulation; therapeutic exercises; modalities; use of supportive and assistive devices; wound care; etc.

• Provide consultation and counseling to patients, families, caregivers, and other service providers related to personal physical disorders.

• Provide comprehensive discharge summaries of services per regulatory and clinical requirements.

Qualifications
