Job summary

Fort Dodge Health and Rehabilitation is seeking a full-time Occupational Therapist to join our in-house rehab program in Fort Dodge, IA. The OT will conduct screenings, develop care plans, and deliver skilled rehabilitation while working within regulatory standards.

The role includes patient and family counseling, documentation, and discharge planning to ensure seamless transitions and high-quality care. Open to new grads and all experience levels.

Qualifications

• OT license is required.

Responsibilities

• Screen and evaluate patients with physical functioning disorders and wound care, and develop care plans.
• Deliver rehabilitative, skilled, and medically necessary treatments per clinical standards.
• Assist with daily activities and patient handling, including standing, walking, lifting, transferring, and repositioning as needed.
• Perform procedures as part of the rehab plan (manual techniques, ambulation, therapeutic exercises, modalities, use of devices, wound care).
• Provide counseling to patients, families, and other providers regarding physical disorders.
• Generate discharge summaries in compliance with regulatory requirements.

Fort Dodge Health and Rehabilitation in Fort Dodge, IA, is a post-acute care facility with long-term care, short-term care, and geriatric services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a dedicated Full Time Occupational Therapist for our in-house rehab program!

KEY RESPONSIBILITIES

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• The role involves regular standing and walking throughout the facility and occasional lifting, transferring, and repositioning of patients and equipment. Physical demands vary throughout the day based on patient care needs.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.

QUALIFICATIONS

• Occupational Therapist license is required.
• Open to Occupational Therapist at all experience levels. New Grads are welcome!
