Role Description

The Physical Therapist role is a contracted position and the PT can choose the size of caseload desired. In-person and telehealth options are available. The Physical Therapist will assess clients’ functional status, develop individualized treatment plans, and provide evidence-based physical therapy interventions to improve mobility, strength, and overall function. Responsibilities include performing evaluations, documenting clinical findings, monitoring progress, and adjusting treatment plans as needed. The role involves close collaboration with other therapy disciplines and services offered by the practice, educating clients and caregivers on home programs, and promoting safety and independence in daily activities. The Physical Therapist will also participate in interdisciplinary meetings and contribute to quality improvement initiatives within the organization.

Qualifications

• Demonstrated knowledge and skills in Physical Therapy and Rehabilitation.
• Ability to deliver high-quality Patient Care across diverse populations.
• Experience or interest in community-based service delivery.
• Foundational understanding of Medicine relevant to musculoskeletal and neurological conditions.
• Current licensure or eligibility for licensure as a Physical Therapist in the state of Colorado.
• Strong assessment, clinical reasoning, and documentation skills.
• Effective communication and collaboration skills within an interdisciplinary team.
• Comfort working in a mobile or variable setting and adapting to client environments.
• Autonomy with getting documentation completed, connecting with clients and providing excellent care.
