Job Juncture is seeking a dedicated Physical Therapist to provide home health services in Colorado. This full-time position focuses on delivering patient-centered therapy in patients’ homes.

You will perform assessments, develop individualized treatment plans, and guide patients through rehab, while collaborating with a multidisciplinary team and maintaining accurate records per regulatory standards.

Qualifications

• Minimum nine months of experience as a physical therapist
• Preferred community/home health experience
• Must possess and maintain CPR certification
