Job summary

Little Diamonds Early Intervention is seeking a dedicated Physical Therapist to join our compassionate team. You will assess, plan, and implement individualized treatment to improve mobility, reduce pain, and promote well‑being across outpatient clinics, hospitals, inpatient units, and home health settings.

You will collaborate with physicians, nurses, therapists, and social workers, document care in EHRs, guide discharge planning, and educate patients and families on injury prevention and home

Qualifications

• Experience across outpatient clinics, inpatient hospital units, home health, and outpatient rehabilitation settings.
• Proficiency with EHR/EMR documentation for accurate patient care records.
• Ability to develop evidence-based treatment plans from thorough assessments and current guidelines.

Responsibilities

• Conduct comprehensive evaluations to identify functional limitations and develop tailored treatment plans.
• Implement interventions such as manual therapy, trigger point therapy, exercise programs, and modalities.
• Collaborate with physicians, nurses, therapists, and social workers to coordinate care and discharge planning.
• Maintain accurate clinical documentation in EHRs in compliance with regulations and standards.
• Manage discharge planning for home health or community services post-treatment.
• Treat diverse populations including geriatric, pediatric, neurology, orthopedics, and outpatient rehab patients.
• Educate patients and families on injury prevention, procedures, exercises, and home care strategies.

Qualifications
