Job summary

Flagship Therapy in Carrabelle, FL is seeking full-time Physical Therapists for our in-house rehab program. You will evaluate patients with functional disorders and develop individualized plans of care within a supportive Ensign-affiliates network dedicated to geriatric recovery.

Key duties include delivering skilled therapy, wound care when needed, documenting discharge summaries, and coordinating with families and other providers to ensure smooth transitions.

Qualifications

• Physical Therapist license is required.
• Open to Physical Therapists at all experience levels. New Grads are welcome!

Responsibilities

• Conduct screening and evaluation of patients with physical functioning disorders and/or wound care.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions.
• Assist with daily activities requiring standing, walking, lifting, transferring, and repositioning of patients and equipment.
• Administer procedures as part of the rehabilitation plan, including manual techniques and therapeutic exercises.
• Provide consultation to patients, families, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries ensuring regulatory and clinical compliance.

Our expanding network of in-house therapy Ensign-affiliate operated healthcare facilities is growing, and we are scouting for excellent therapists to join us at Carrabelle Post-Acute and Rehab in Carrabelle, FL. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking dedicated Full Time Physical Therapists for our in-house rehab program!

Key Responsibilities

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• The role involves regular standing and walking throughout the facility and occasional lifting, transferring, and repositioning of patients and equipment. Physical demands vary throughout the day based on patient care needs.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.
