Flagship Therapy is seeking a PRN Physical Therapist for Shores Post-Acute and Rehab in Port St Joe, FL. You will join an in-house rehab team serving geriatric patients, delivering skilled therapies and developing comprehensive care plans.

The role emphasizes patient evaluation, treatment delivery, and collaboration with a multidisciplinary team in a post-acute setting. New grads are welcome to apply, with opportunities to grow within the in-house program.

Responsibilities

SHORES POST-ACUTE AND REHAB – PORT ST JOE, FL

Our expanding network of in-house therapy Ensign-affiliate operated healthcare facilities is growing, and we are scouting for excellent therapists to join us at Shores Post-Acute and Rehab in Port St Joe, FL. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a dedicated PRN Physical Therapist for our in-house rehab program!

Key Responsibilities:

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.


• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.


• The role involves regular standing and walking throughout the facility and occasional lifting, transferring, and repositioning of patients and equipment. Physical demands vary throughout the day based on patient care needs.


• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.


• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.


• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.

Qualifications

• Physical Therapist license is required.
