Job summary

Cranford Park Care in Cranford, NJ is seeking a licensed Physical Therapist to evaluate, plan, and implement patient-centered rehabilitation. You will assess strength, mobility, and functional needs, develop individualized treatment plans, and administer therapies per physician orders to improve outcomes.

You will also mentor assistants, maintain safe equipment, document progress, and participate in discharge planning and care team meetings to ensure seamless patient care.

Job description

Responsibilities

• Evaluate patients individually to determine strength and mobility issues, pain problems, and treatment needs
• Develop treatment plans for patients based on their care needs and current physical abilities
• Treat patients following a physician’s orders and using various therapeutic techniques to resolve strength, mobility, and functionality issues
• Add notes to patient files regarding performance, progress, and treatment needs
• Demonstrate the proper methods for using therapeutic tools and mobility aids, such as crutches, wheelchairs, and walking canes
• Instruct patient's family or nursing staff in follow-through programs
• Maintain equipment and work area in a safe and clean condition
• Supervises Physical Therapy Assistants according to organization policy and state regulations
• Participate in care plan meetings
• Participates in discharge planning for patient

Qualifications

• Current license from the NJ State Board of Physical Therapy
