Job Summary

  1. Plan, prepare, and carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, and prevent physical dysfunction in patients.
  2. Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.
  3. Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
  4. Administer manual exercises, massage, or traction to help relieve pain, increase patient strength, or decrease or prevent deformity.
  5. Instruct patient and family in treatment procedures to be continued at home.
  6. Confer with the patient, medical practitioners, and appropriate others to plan, implement, and assess the intervention program.
  7. Review physician's referral and patient's medical records to help determine diagnosis and physical therapy treatment required.
  8. Record prognosis, treatment, response, and progress in patient's chart or enter information into computer.
  9. Obtain patients' consent to proposed interventions.
  10. Discharge patient from physical therapy when goals or projected outcomes have been attained and provide for appropriate follow-up care or referrals.
  11. Supervise, train, and assess therapy students during onsite clinical affiliations.

Non-Essential Accountabilities
  • Clinical Education: Serve as a clinical instructor after gaining one year of experience and if a student is available.
  • Assist with clinical education of students if not the primary Clinical Instructor.
  • Staff Education: Provide formal presentations of evidence-based information.
  • Attend institutional in-services, staff meetings, and continuing education programs.
Patient/Employee Safety Accountabilities
  • Participate in safety initiatives for patients and employees.
  • Demonstrate awareness of safety in daily responsibilities.
  • Validate annual competencies required for the position.
Operations
  • Perform therapy examination, treatment planning, and implementation.
  • Apply principles of logic and scientific method in practice.
  • Ensure full compliance with licensure, ethical, and legal standards.
  • Maintain therapy documentation and participate in compliance audits.
  • Meet productivity expectations.
  • Wear Health System ID as per policy.
Qualifications
  • Bachelor's Degree in Physical Therapy required; Master’s preferred.
  • Previous healthcare experience preferred.
  • Physical Therapist license required.
  • Basic Life Support / CPR certification required.
