Colorado Visiting Nurse Association has an immediate opening for a Physical Therapist.
Location: Commerce City, Brighton, Thornton service areas
Status: Part-time
Schedule: M-F with rotating on call weekends every 4-6 weeks
Pay is based off a Per Visit Rate scale.
Supplemental Pay: Based on position, schedule and/or availability: Paid Mileage
Mileage and Expense Reimbursement: Sixty-two and a half cents per mile – one of the highest in the industry!

REWARDING WORK YOU WILL DO:
Responsible for performing physical therapy, evaluating client needs and conducting case management for patients under the direction of a licensed physician.
Provides initial and ongoing assessments to determine level of function, including OASIS assessments at appropriate time point, safety evaluations and recommendations for home modifications and Home Health Aide training/supervision.
Performs specific therapeutic exercises as needed.
The procedures required by the therapist may include (but are not limited to): ultrasound treatment, application of heat and cold, massage, gait training, muscle stimulation, assistance and training in cardio-pulmonary exercise, mobilization, assistance and training with ambulation, assistance and training with balance, range of motion exercises, patient transfer, strengthening techniques, electrotherapy, prosthetic training, and adaptive equipment.
Advises and consults with the family, physician and other agency personnel, including discharge planning.
Provides supervision over the Physical Therapy Assistants and Certified Nursing Assistants.
WHAT WE ARE GOING TO LOVE ABOUT YOU:
Minimum Education: Graduation from a four-year college or university, including completion of a physical therapy education program.
Minimum Experience: At least one year of physical therapy experience required, with two years of SNF, hospital or inpatient rehab preferable. Home health experience also preferred. Other requirements may vary by case or department needs.
Required License: Must have active Colorado PT license. Current CPR certification based on individual affiliate requirements. Valid Colorado driver’s license and proof of automobile insurance in accordance with state statute.
PHYSICAL REQUIREMENTS:
Ability to lift/push/pull minimum of 35 lbs
Colorado Visiting Nurse Association is affiliated with Care Synergy, a nonprofit network of community-based hospice and palliative care organizations serving Colorado’s Front Range and the Region’s Largest Home Health, Hospice, and Palliative Care Network. Care Synergy provides mission support services to better equip Colorado Visiting Nurse Association to operate as a distinct and independent organization while working together to share best practices and serve more Coloradans along the Front Range.
