Job summary

BrightStar Care – Concord, Lexington & Woburn is seeking a licensed Physical Therapist to join our home-based team on a part-time / per diem basis. You will assess clients in their homes, design personalized care plans, and deliver skilled PT interventions to improve strength, balance, mobility, and safety.

You will educate families, collaborate with a multidisciplinary team, and document progress using a mobile EMR.

Qualifications

• Active MA PT license required.
• DPT or MPT from an accredited program.
• CPR certification required.
• 1+ year experience preferred; new grads welcome.

Responsibilities

• Perform in-home PT evaluations and create personalized care plans (home visits).
• Provide skilled PT interventions to improve strength, balance, mobility and safety.
• Educate clients and families on exercises and home safety.
• Collaborate with a multidisciplinary care team.
