Qualifications

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Deliver evidence-based physical therapy interventions.
• Improve patients' mobility, strength, balance, and overall function.
• Collaborate with physicians, nursing staff, therapy team members, and families.
• Document evaluations, progress notes, and discharge plans accurately and timely.
• Educate patients and caregivers on treatment plans and home exercise programs.
• Participate in interdisciplinary care planning meetings.

What You'll Do
