Whitney Oaks Care Center in Carmichael, CA is seeking a compassionate Physical Therapist to join our rehabilitation team. This full-time role offers an hourly pay range of $68.00–$70.00 and a sign-on bonus for full-time hires, with relocation assistance available.

You will evaluate residents, develop personalized treatment plans, deliver evidence-based therapies, and collaborate with physicians, nursing staff, and families to improve mobility and independence in a post-acute and long-term care

Competenze

• Current California Physical Therapist (PT) license in good standing.
• Graduate of an accredited Physical Therapy program.
• Strong clinical assessment and treatment skills.
• Excellent communication and interpersonal abilities.
• Experience in skilled nursing, post-acute, rehabilitation, or long-term care preferred.
• New graduates with strong clinical training are encouraged to apply.

Mansioni

• Evaluate residents and develop individualized treatment plans.
• Deliver evidence-based physical therapy interventions.
• Improve patients' mobility, strength, balance, and overall function.
• Collaborate with physicians, nursing staff, therapy team members, and families.
• Document evaluations, progress notes, and discharge plans accurately and timely.
• Educate patients and caregivers on treatment plans and home exercise programs.
• Participate in interdisciplinary care planning meetings.
