Stern at Home Therapy is hiring Physical Therapists to provide home-based care in the Buffalo, NY area, serving a predominantly geriatric population. Both full-time and part-time positions are available, allowing clinicians to build a primary or supplemental caseload within ZIP codes 14201–14240.

Clinicians have autonomy to develop individualized care plans while our administrative team handles referrals, scheduling, authorizations, and coordination.

Qualifications

• Active Physical Therapist license in New York.
• Graduate of an accredited Physical Therapy program.
• Reliable transportation.
• Strong clinical, communication, and time‑management skills.
• Ability to work independently in a home health setting.

Responsibilities

• Evaluate and treat a geriatric patient population in patients' homes.
• Develop individualized plans of care with clinical autonomy; support staff assists with referrals and scheduling.
• Perform PT evaluations and develop plans of care.
• Provide one-on-one PT treatments in patients' homes.
• Help patients improve strength, balance, mobility, endurance, and functional independence.
• Provide gait training, transfer training, fall-prevention strategies, and home exercise programs.
• Recommend devices, durable medical equipment, and home safety modifications when appropriate.
• Complete accurate and timely clinical documentation.
• Communicate with physicians, caregivers, families, and the interdisciplinary care team.
• Coordinate with administration to develop a Buffalo-area caseload.

Physical Therapist (PT) – Home Health | Buffalo, NY | Full-Time & Part-Time | $45–$75 Per Visit | 3 Openings.

Stern at Home Therapy is expanding throughout the Buffalo, NY area and is hiring three Physical Therapists (PTs) to support our continued growth.

We are hiring in the following Buffalo, NY areas: ZIP codes 14201 through 14240. This lets clinicians build a caseload closer to home rather than covering the entire region.

We are hiring for Full-Time and Part-Time positions, making this a strong opportunity whether you're building a primary caseload or adding a part-time caseload around an existing position.

What You'll Do:

• As a Physical Therapist with Stern at Home Therapy, you'll evaluate and treat a primarily geriatric patient population in their homes.
• You'll have the clinical autonomy to develop individualized plans of care while our administrative team supports you with referrals, scheduling, authorizations, and patient coordination.

Compensation:

• Full-Time – Minimum 16 Patients.

W-2 Tiered Per-Visit Option:

• $45–$60 per follow-up visit.
• $65 per 4-unit evaluation.
• Visit-based pay brackets: Visits are paid according to the bracket they fall into. The higher rate applies only to visits within the higher bracket.

1099 Tiered Per-Visit Option:

• $60–$70 per follow-up visit.
• $75 per evaluation.
• Visit-based pay brackets: Visits are paid according to the bracket they fall into. The higher rate applies only to visits within the higher bracket.

Part-Time – Minimum 10 Patients:

W-2:

• $45–$55 per visit, based on experience.
• $65 per evaluation.

1099:

• $62 per follow-up visit.
• $75 per evaluation.

Full-Time W-2 Benefits:

• 401(k) with annual match – employer match starts at 1.5% and increases up to 7%.
