Stern at Home Therapy is seeking a licensed Physical Therapist (PT) to provide one-on-one home health care in the South Salem, NY area. Part‑time W‑2 with benefits or flexible PRN opportunities are available, tailored to your schedule and caseload.

You will evaluate and treat a geriatric population in patients’ homes, with clinical autonomy and strong administrative support to coordinate scheduling and logistics.

Qualifications

• An active Physical Therapist (PT) license in New York is required.
• Reliable transportation is required.
• Ability to work independently in a home-based setting.
• Home health experience is preferred but not required.

Responsibilities

• Perform physical therapy evaluations and develop individualized plans of care.
• Provide one-on-one treatments based on each patient's functional needs and goals.
• Manage individualized treatment plans with clinical autonomy.
• Help patients improve mobility, strength, balance, safety, and overall function.
• Complete accurate and timely clinical documentation.
• Communicate with patients, families, physicians, and members of the clinical team.
• Coordinate with our administrative team regarding scheduling, patient care, and logistics.
• Build and manage your caseload based on your availability and preferred coverage area.

Physical Therapist (PT) – Home Health | South Salem, NY | PRN & Part-Time | $65–$85 Per Visit

Stern at Home Therapy is hiring a Physical Therapist (PT) to provide one-on-one care to patients in their homes in and around South Salem, NY.

Choose between part-time W-2 employment with benefits or a flexible PRN opportunity based on your availability.

This opportunity is ideal for a licensed Physical Therapist looking to build a steady part-time caseload or add flexible PRN visits to an existing schedule without the double-booking, productivity quotas, and clinic burnout common in traditional outpatient settings.

Treat one patient at a time, manage your availability, and provide individualized care with support from an established clinical and administrative team.

What You'll Do:

As a Physical Therapist with Stern at Home Therapy, you will evaluate and treat a primarily geriatric patient population in their homes throughout the South Salem area.

Responsibilities include:

• Perform physical therapy evaluations and develop individualized plans of care.
• Provide one-on-one treatments based on each patient's functional needs and goals.
• Manage individualized treatment plans with clinical autonomy.

Compensation:

Part‑Time W‑2:

• $65 per follow-up visit.
• $75 per 4-unit evaluation.

PRN:

• $72 per 4-unit follow-up visit.
• $85 per evaluation.
