Job summary

Stern at Home Therapy is seeking a PRN Physical Therapist (PT) to provide one-on-one care to geriatric patients in Ocean County, NJ, including Barnegat. This role supports flexible scheduling and the ability to supplement an existing workload while maintaining autonomy in home-based care.

The PT will evaluate and treat at patients’ homes with administrative support handling referrals, scheduling, and coordination. A NJ license and independent practice are required for success in this position.

Qualifications

• Active Physical Therapist license in New Jersey.
• Graduate of an accredited Physical Therapy program.
• Strong clinical, communication, documentation, and time-management skills.
• Ability to work independently in a home-based environment.

Responsibilities

• Perform PT evaluations and develop individualized plans of care.

Physical Therapist (PT) – Home Health | Ocean County & Barnegat, NJ | PRN

Stern at Home Therapy is hiring a PRN Physical Therapist (PT) to provide one-on-one care to patients throughout Ocean County, NJ, including the Barnegat area.

This opportunity is ideal for a Physical Therapist looking to supplement an existing schedule, maintain a small caseload, or add flexible home health work while maintaining control over when and where they see patients.

With a minimum commitment of just 5 patients, our PRN option provides flexibility while allowing therapists to build a geographically concentrated caseload within their preferred areas of Ocean County.

What You'll Do:

As a Physical Therapist with Stern at Home Therapy, you'll evaluate and treat a primarily geriatric patient population in their homes.

You'll maintain clinical autonomy over evaluations and treatment plans, while our administrative team supports referrals, scheduling, authorizations, and patient coordination.

Responsibilities include:

• Perform physical therapy evaluations and develop individualized plans of care.
• Provide one-on-one physical therapy treatments in patients’ homes.
• Help patients improve strength, balance, mobility, endurance, and functional independence.
• Provide gait training, transfer training, therapeutic exercise, and fall-prevention strategies.
• Recommend assistive devices, durable medical equipment, and home safety modifications when appropriate.
• Educate patients and caregivers on exercises, mobility strategies, and safety.
• Monitor patient progress and modify treatment plans as clinically appropriate.
• Complete accurate and timely clinical documentation.
• Communicate with physicians, caregivers, families, and interdisciplinary care team members.

Compensation:

PRN 1099 – Minimum 5 Patients:

• $72 per follow-up visit.
• $85 per evaluation.
• Fixed per-visit compensation with no tiered volume model.
• PRN positions are 1099 independent contractor roles only.

Qualifications:
