Elara Caring is seeking a dedicated Physical Therapist to deliver exceptional home-based care across Kentucky. You will evaluate patients, create individualized treatment plans, and provide therapy in patients' homes, while coordinating with physicians and care teams to ensure optimal outcomes.
The role requires graduation from an APTA-approved program, an active unrestricted PT license for the state, and at least 1 year of healthcare facility experience.
