Home&Care Home Health is seeking a dedicated Physical Therapist to join our team in Billings, MT, providing skilled PT services in patients' homes. The role offers full-time stability with meaningful relationships built with patients and families to promote independence in the comfort of home.

Responsibilities include conducting assessments, developing personalized treatment plans, and supervising assistants, while maintaining thorough documentation and strong communication with care teams

Qualifications

• Strong communication, organizational, and leadership skills.
• CPR certification.
• Current Montana PT license.
• Degree from an accredited Physical Therapy program (APTA-approved)

Responsibilities

• Provide skilled physical therapy treatments to patients with injuries, illnesses, and functional limitations to help them reach their highest potential.
• Conduct assessments, record findings, and develop individualized treatment programs with measurable goals.
• Design and implement rehabilitation programs aligned with physician plans and patient goals, adapting as needed for optimal results.
• Collaborate with physicians and the care team to update and refine treatment plans.
• Supervise and provide guidance to Physical Therapy Assistants to ensure high-quality care delivery.
• Maintain timely, accurate, and thorough documentation of patient care and progress.
• Communicate effectively with patients, families, and team members, updating care plans as needed.
