Job summary

SNFOut in Billings is seeking a full-time Physical Therapist to provide skilled treatments in patients' homes, assess functional needs, and develop individualized plans with measurable goals. You will document progress and adapt plans as needed to optimize outcomes.

You will collaborate with physicians and the care team, supervise Physical Therapy Assistants, and communicate clearly with patients and families to support independence, comfort, and quality of life.

Qualifications

• Degree from an accredited Physical Therapy program (APTA-approved).
• Current Montana PT license and CPR certification.
• Strong communication, organizational, and leadership skills.

Responsibilities

• Provide skilled physical therapy treatments to patients with injuries, illnesses, and functional limitations to help them reach their highest potential.
• Conduct assessments, record findings, and develop individualized treatment programs with measurable goals.
• Design and implement rehabilitation programs aligned with physician plans and patient goals.
• Collaborate with physicians and the care team to update treatment plans.
• Supervise and guide PTAs to ensure high-quality care.
• Maintain timely, accurate documentation of patient care and progress.
• Communicate effectively with patients, families, and team members, reporting progress and updating care plans.

Physical Therapist Qualifications:
