Best Home Health Providers Inc. is seeking a per diem Physical Therapist to deliver skilled PT in patients' homes across the Bay Area and beyond. You will assess mobility, design plans of care, educate families, and collaborate with physicians to support recovery.

A CA license and CPR certification are required, with at least 2 years of PT experience; a valid driver's license is needed for travel. Join a compassionate, patient-centered team offering flexible scheduling, paid leave, and growth

Formación

• Bachelor's or Master’s degree in Physical Therapy from an accredited program.
• Active California Physical Therapy license.
• Minimum 2 years of PT experience; home health background is a plus.
• Current CPR certification with AHA.

Responsabilidades

• Evaluate patients’ functional status, mobility, muscle strength, gait, and ROM to develop individualized treatment plans.
• Deliver physical therapy services in accordance with the physician’s plan of care and California’s Physical Therapy Practice Act.
• Educate patients and their families or caregivers on therapeutic exercises, equipment use, and recovery goals.
• Collaborate with physicians and the care team—reporting changes in patient condition and participating in case conferences.
• Supervise Physical Therapy Assistants and, when PT is the sole skilled service, oversee Home Health Aide care bi-weekly.
• Manage documentation including clinical progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals as appropriate.
• Perform other duties as required for patient services.

Please note that this role is per diem with a rate of $160 - $180 per visit.
