Job summary

Chestnut Health in California is seeking a Nurse Practitioner to join our Physiatry team. In a patient-centered environment you will develop rehabilitation plans and coordinate care with therapists and nurses to improve function and independence.

You will conduct assessments, document evaluations, monitor progress, and educate patients and families about the rehabilitation process and post-discharge care. New graduates are encouraged to apply.

Qualifications

Current active NP license in the state of employment.
Current DEA license upon start date.
ANCC or AANP certification.
Interest in Physiatry/PM&R.
Orthopedic or Skilled Nursing experience preferred.
Desire to work in rehabilitation medicine.

Responsibilities

Collaborate with therapists, nurses, and other healthcare professionals to develop treatment plans.
Conduct patient assessments, histories, and examinations to determine rehabilitation goals.
Monitor progress and adjust treatment plans to optimize outcomes.
Participate in multidisciplinary meetings and educational activities.
Maintain accurate documentation in electronic medical records.
Educate patients and families about rehabilitation and post-discharge care.
