American Physiatry is seeking a compassionate Nurse Practitioner to join our healthcare team in skilled nursing facilities, focusing on pain control and functional rehabilitation. You will work closely with physical therapy, manage a patient caseload independently, and contribute to high-quality, evidence-based care.

Qualifications

• Licensed Nurse Practitioner with PA certification.
• Experience working independently while managing your own caseload.
• Submit accurate and thorough clinical documentation promptly, ensuring compliance with company policies and regulatory requirements.

Responsabilités

• Conduct thorough assessments of patients, identifying and managing medical conditions under the supervision of a Physiatrist.
• Develop and implement individualized care plans to improve patients' function and rehabilitation.
• Collaborate with the healthcare team to ensure comprehensive care, including medication management and therapy recommendations.
• Educate patients and their families on treatment plans, health maintenance, and preventive care.
• Monitor and evaluate patient progress, adjusting care plans as necessary to achieve optimal outcomes.
