• Conduct thorough patient assessments to determine rehabilitation goals and interventions
• Monitor patient progress and adjust treatment plans to optimize outcomes
• Participate in multidisciplinary meetings and educational activities
• Maintain accurate documentation in electronic medical records
• Provide education to patients and families regarding rehabilitation and post-discharge care
