Job summary

Chestnut Health in Jacksonville, FL seeks a Nurse Practitioner to join our Physiatry team. You will collaborate with therapists and nurses to design comprehensive rehab plans and conduct thorough assessments to drive functional outcomes.

The role emphasizes patient education, documentation in EMR, and ongoing multidisciplinary learning within a patient-centered environment. New graduates are encouraged to apply and growth opportunities exist within the rehab continuum.

Qualifications

• Eligible to work in the United States.
• Actively licensed to practice in the state of employment preferred.
• Current Controlled Substance License by start date preferred.
• Current DEA License by start date preferred.
• ANCC or AANP certification preferred.
• Interest in Physiatry / Physical Medicine and Rehabilitation is a plus.
• Experience in orthopedics is a plus.
• Experience in physiatry is a plus.
• Experience in skilled nursing is a plus.

Responsibilities

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs.
• Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions.
• Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence.
• Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills.
• Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems.
• Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care.
