Job summary

Lighthouse Care Center of Augusta, part of Universal Health Services, is seeking a PHP Therapist to join our outpatient team in Augusta, GA. The role focuses on coordinating patient-centric therapeutic strategies and ensuring high-quality, cost-effective care in compliance with regulations.

You will perform comprehensive assessments, oversee treatment plans, coordinate discharge, and collaborate with referrals, families, and community resources as part of a multidisciplinary team.

Qualifications

• Master's degree in counseling, social work or related field.
• Licensure required (LAPC, LPC, LMSW, LCSW, etc).
• 1–3 years’ experience in mental health.

Responsibilities

• Comprehensive assessment of patients in outpatient setting.
• Oversee individualized treatment plans, including nursing care, medication management, group therapy, educational and family therapy.
• Make referrals to community resources as appropriate.
• Coordinate discharge planning through the medical staff.
• Function as a member of the multidisciplinary team and participate in community relations.
• Participate in continuous performance improvement activities.
• Communicate with outpatient referral sources, families/guardians, and aftercare providers.

The Partial Hospitalization Program (PHP) Therapist is a key member of the Lighthouse Clinical Services Team who will integrate and coordinate a patient centric therapeutic strategy with a keen focus on patient care; ensuring that delivery of high-quality and cost-effective treatment is consistent with the mission, vision, and values of Universal Health Services and in accordance with government regulation, licensing and accreditation requirements. The PHP Therapist is responsible for comprehensive assessment of persons whose lives are disrupted or complicated by mental illness, behavioral disturbance, substance issues or inability to function in the outpatient setting.

Job Responsibilities

• Makes referrals to community resources as appropriate.
