• Make referrals to community resources as appropriate.
• Coordinate discharge planning through the medical staff.
• Function as a member of the multidisciplinary team and participate in the organization’s community relations program.
• Participate in continuous performance improvement activities.
• Communicate with outpatient referral sources, families/guardians, and outpatient aftercare providers.

Responsibilities

The PHP Therapist is a key member of Lighthouse Clinical Services Team. The role focuses on integrating and coordinating a patient-centric therapeutic strategy to deliver high-quality, cost-effective treatment in accordance with the mission, vision, and values of Universal Health Services and applicable government regulation, licensing, and accreditation requirements. The PHP Therapist conducts comprehensive assessments and provides treatment planning and coordination for patients whose lives are disrupted by mental illness, behavioral disturbance, substance issues, or functional impairment in the outpatient setting.

• Comprehensive assessment of patients in outpatient settings.
