Qualifications

Responsibilities

• Conducts individual and group therapy sessions to educate patients about psychological, emotional, or substance abuse issues.
• Active involvement in treatment planning and discharge planning processes.
• Provides family session counseling to involve family and support groups in care.
• Communicates with clients, families, and outside referral sources to coordinate care.
• Works with interdisciplinary team including physicians, nursing and utilization review staff.
