Job summary

You will implement performance improvements from patient outcomes reviews, maintain documentation, and collaborate with providers to optimize care for diverse patient populations in Minnesota.

Qualifications

• Registered Nurse with Minnesota license and current practice knowledge of chronic disease management.
• Experience coaching health behavior change and guiding self-management.
• Strong communication with patients and care teams across settings.

Responsibilities

• Provide telephone-based services including outreach, evaluation, engagement, and education.
• Coordinate care and integrate services across care continuum with providers and family.
• Support self-management and performance improvement based on outcomes review.
• Facilitate communication between patients, health providers, and community resources.
