Qualifications

• Registered Nurse with minimum 3 years' experience in chronic disease management.
• CDCES certification preferred.
• Experience educating, counseling and coaching patients in various formats.

Responsibilities

• Facilitates chronic condition management and prevention programs.
• Develops individualized interventions to improve health outcomes.
• Educates and counsels patients on disease prevention and management.
• Coordinates with healthcare team to optimize care and patient engagement.
• Conducts outreach and provides face-to-face, virtual, and telephonic coaching.
