Job summary

Endeavor Health in Elmhurst is seeking a Pharmacy Technician Med Reconciliation to collect home medication histories for patients admitted to the hospital, primarily in the Emergency Department, under supervision of a registered pharmacist.

You will interview patients, contact pharmacies, document current meds, and support safe transitions of care. Requires Illinois Pharmacy Technician license, High school diploma, and potential certification within 90 days.

Qualifications

• High school diploma or GED required.
• Current License as a Pharmacy Technician in the State of Illinois required.
• Within 90 days of hire, obtain CPhT certification or equivalent (PTCB/ExCPT) or have an Illinois Technician license with Student designation.

Responsibilities

• Collect Home Medication History for patients admitted to the hospital.
• Interview patients, identify recent outpatient meds, and contact pharmacies.
• Document collected information accurately under supervision of a registered pharmacist.
