Job summary

Endeavor Health in Elmhurst, IL, is seeking a Pharmacy Technician Med Reconciliation to support patient care in our hospital setting. You will collect Home Medication Histories for patients admitted to the Emergency Department and coordinate with pharmacists to verify accuracy.

This is a full-time role (40 hours/week) with rotating evening shifts from 11am–9:30pm or 12pm–10:30pm; four shifts per week with every third weekend and rotating holidays required.

Qualifications

• High school diploma or GED is required.
• Current Illinois Pharmacy Technician license is required.
• Candidates may also meet requirements via CPhT within 90 days, or PTCB/ExCPT pending, or Illinois license with Student designation.

Responsibilities

• Collect Home Medication History for patients admitted to the hospital, interviewing patients and contacting pharmacies to document meds accurately.
• Work primarily in the Emergency Department and may assist on inpatient floors, always under the supervision of a registered pharmacist.

Hourly Pay Range

$20.99 - $32.53 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

What You Will Do

Assist in the process of collecting Home Medication History of patients being admitted to the hospital. This will require using the resources provided to identify recent outpatient medication dispenses, interviewing the patient, contacting pharmacies, and accurately documenting the collected information. This work will primarily be conducted in the Emergency Department but may also include inpatient floors. Works only under the supervision of a registered pharmacist

What You Will Need

• Education (Required):
• High school diploma or GED equivalent
• Certifications (Required):
• Current License as a Pharmacy Technician in the State of Illinois
• One of the following must be met:
• Current Pharmacy Technician Certification or must obtain CPhT (Certified Pharmacy Technician) within 90 days of hire, or
• Successfully passed the Pharmacy Technician Certification Board (PTCB) or the Exam for the Certification of Pharmacy Technicians (ExCPT) examination pending certification, or
• Current Pharmacy Technician license with “Student” designation issued by the State of Illinois
