Edward Elmhurst Health’s EMH Main Hospital in Elmhurst is seeking a Pharmacy Technician Med Reconciliation to join our team. This full-time role (40 hours weekly) supports admission workflows by collecting patient home medication histories and documenting findings under pharmacist supervision.

You’ll interview patients, contact pharmacies, and verify medications, with evening rotating shifts (11am–9:30pm or 12pm–10:30pm) and every third weekend.

Qualifications

• High school diploma or GED required.
• Illinois Pharmacy Technician license required.
• CPhT/PTCB/ExCPT eligibility within 90 days of hire or license with Student designation issued by IL

Responsibilities

• Assist in collecting Home Medication History for patients admitted to the hospital.
• Interview patients and contact pharmacies to verify medications.
• Document collected information accurately under pharmacist supervision.
• Work primarily in the Emergency Department with potential inpatient floor duties.

## Pharmacy Technician Medicine ReconciliationApply: EMH Main Hospital Elmhurst: Full time: Posted Yesterday: R47170**Hourly Pay Range:**$20.99 - $32.53 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.**Position Highlights:*** Position: Pharmacy Technician Med Reconciliation* Location: Elmhurst Hospital* Full Time: 40 hours/week* Hours: Rotating Evenings 11am - 9:30pm or 12pm-10:30pm. Each shift is 10 hours; 4 shifts per week with every third weekend and rotating holidays required**What you will do:**Assist in the process of collecting Home Medication History of patients being admitted to the hospital. This will require using the resources provided to identify recent outpatient medication dispenses, interviewing the patient, contacting pharmacies, and accurately documenting the collected information. This work will primarily be conducted in the Emergency Department but may also include inpatient floors.
