Job summary

Endeavor Health is seeking a Pharmacy Technician Med Reconciliation at Elmhurst Hospital in Elmhurst, IL. You will support the medication history collection for patients admitted to our emergency department, working under a registered pharmacist.

The role requires an Illinois Pharmacy Technician license (or eligible) and a High school diploma. You will join a team dedicated to safe, high-quality patient care and collaborative practice.

Qualifications

• High school diploma or GED equivalent.
• Illinois Pharmacy Technician license required.
• Must obtain CPhT within 90 days of hire, or pass PTCB/ExCPT, or hold a State of Illinois Student license.

Responsibilities

• Assist in collecting Home Medication History for patients admitted to the hospital.
• Identify recent outpatient dispenses, interview patients, contact pharmacies.
• Document collected information accurately; work mainly in the Emergency Department under supervision of a pharmacist.
