Job summary

Clearway Health in Washington, D.C. seeks a Pharmacy Patient Liaison I to bridge patients and pharmacists in our specialty pharmacy program. You will support complex therapies, drive enrollments, and enable clinical teams to deliver outstanding patient outcomes.

You will engage with patients, providers, and care teams, ensuring seamless medication adherence and optimal care transitions. This role emphasizes accuracy, confidentiality, and proactive problem solving in a fast-paced environment.

Qualifications

• 2+ years of direct pharmacy experience required.
• Active and unrestricted Pharmacy Technician license with the Board of Pharmacy in the respective state of application is required.
• Active and unrestricted National Certification verified by PTCB or ExCPT.
• Excellent verbal and written English communication skills.
• Ability to use EMR and Microsoft Office applications (Excel/Word/Outlook).
• Knowledge of outpatient pharmacy operations and specialty pharmacy basics is a plus.

Responsibilities

• Develop and nurture patient relationships to expand enrollment in Clearway Health Specialty Pharmacy services.
• Perform enrollment functions and follow up on leads for new business.
• Support patient adherence and retention through continuous engagement and care coordination.
• Build and maintain relationships with providers, care teams, and ancillary staff to ensure continuity of care and enrollment.
• Collaborate with internal teams and external partners to optimize patient transitions from clinic to pharmacy.
• Assist in training and supporting new hires for day-to-day operations.
