Job summary

Clearway Health is seeking a Clinical Pharmacist to optimize outcomes for patients on chronic and specialty therapies. You will review medications, educate patients and staff, and coordinate with physicians, nurses, and payors to support therapy initiation and continuity.

The role emphasizes ensuring safety, efficacy, and adherence, with active participation in prior authorization, outcomes monitoring, and program development to improve workflow and patient satisfaction.

Qualifications

• PharmD or bachelor’s degree from an accredited College of Pharmacy is required.
• Completion of PGY-1 Pharmacy Residency or 3+ years in direct patient care is required.
• PGY-2 Ambulatory Care or Specialty Pharmacy Residency/Fellowship preferred.
• Active Pharmacist licensure in the state of application (or ability to acquire at hire).
• Must obtain State Board of Pharmacy Preceptor’s license within one year of hire.
• Demonstrated expertise in medications, drug interactions, and disease states.

Responsibilities

• Conduct comprehensive medication therapy reviews for patients on specialty medications.
• Provide disease-specific counseling and education to enhance patient engagement.
• Collaborate with prescribers, nurses, case managers, and payors to initiate/continue specialty therapies.
• Support prior authorization, appeals, and benefits verification for timely access.
• Monitor clinical outcomes and adherence, report interventions for quality improvements.
• Maintain documentation and report outcomes to support audits and accreditation.
