Job summary
HCA Healthcare Trident Hospital is seeking a Pharmacy Tech specializing in medication history to support the admission process. You will collect and verify patient medication histories, document details in the electronic health record, and coordinate with pharmacists for cosignature.
Under supervision, you will assist with discharge counseling identifications and prepare education materials for pharmacists. The role may require rotating weekend/evening/Holiday shifts and other hours as needed.
Qualifications
- Must have High School Diploma or GED.
- Intern Pharmacist or Certified Pharmacy Technician license required.
- Pharmacy technician license (PharmTech/CPhT) preferred.
Responsibilities
- Perform medication histories for new hospital admissions under supervision of the clinical pharmacist.
- Verify and update medication history in the electronic health record.
- Document medication histories in CPW/VigiLanz and send to pharmacist for cosignature.
- Assist the medication reconciliation pharmacist with discharge counseling identifications.
- Prepare education materials for the medication reconciliation pharmacist.
- Shift may include weekends, evenings, holidays, on-call, and additional hours.
