You will verify allergy data, document histories in the EHR, and assist with reconciliation and patient education materials as needed.

Qualifications

High School diploma or GED required.
PHARM-Intern, CPhT, or PharmTech license required.
Experience handling medication histories is a plus.

Responsibilities

Perform medication histories for new hospital admissions.
Verify/update allergy, adverse drug reaction, and medication data in the EHR.
Assist the medication reconciliation pharmacist and prepare education materials as needed.
