Endeavor Health is seeking a Pharmacy Technician Med Reconciliation to assist in collecting patient home medication histories for admissions at Highland Park Hospital. You will interview patients, verify outpatient medications with pharmacies, and document findings under pharmacist supervision in a busy ED and inpatient areas.

Typical schedule is full-time, 40 hours weekly, with 10-hour shifts Monday through Friday and rotating weekends.

Responsibilities

• Assist in the process of collecting Home Medication History of patients being admitted to the hospital. This will require using the resources provided to identify recent outpatient medication dispenses, interviewing the patient, contacting pharmacies, and accurately documenting the collected information.
