Job summary

JobRx, Inc. seeks a Pharmacy Technician Med Reconciliation to support Home Medication History collection for patients admitted to Highland Park Hospital in Highland Park, IL. You will interview patients, coordinate with pharmacies, and document findings under a registered pharmacist's supervision.

The role is full-time (40 hours/week) with Monday–Friday shifts, 10 hours each, and rotating every third weekend. The position offers a competitive hourly pay range and benefits.

Qualifications

Education (Required): High school diploma or GED.
Certifications (Required): Current Illinois Pharmacy Technician license or eligible for CPhT within 90 days, or PTCB/ExCPT pending certification, or Illinois license with 'Student' designation.

Responsibilities

Assist in collecting Home Medication History for patients admitted to the hospital.
Interview patients, contact pharmacies, and accurately document collected information.
Work primarily in the Emergency Department under the supervision of a registered pharmacist.
