Job summary

Endeavor Health is seeking a Pharmacy Technician Med Reconciliation to assist in collecting patient home medication histories for admissions at Highland Park Hospital. You will interview patients, verify outpatient medications with pharmacies, and document findings under pharmacist supervision in a busy ED and inpatient areas.

Typical schedule is full-time, 40 hours weekly, with 10-hour shifts Monday through Friday and rotating weekends.

This will require using the resources provided to identify recent outpatient medication dispenses, interviewing the patient, contacting pharmacies, and accurately documenting the collected information.

Hourly Pay Range:

$20.99 - $32.53 - The hourly pay rate offered is determined by a candidate’s expertise and years of experience, among other factors.

Position Highlights:

• Location: Highland Park Hospital
• Full Time: 40 hours/week
• Hours: Monday-Friday, 10 Hour Shifts, 4 Shifts each Week (11am - 9:30pm, 12pm - 10:30pm), Rotating Every 3rd weekend (12pm - 10:30pm)
• Required Travel: No

What you will do:

Assist in the process of collecting Home Medication History of patients being admitted to the hospital. This will require using the resources provided to identify recent outpatient medication dispenses, interviewing the patient, contacting pharmacies, and accurately documenting the collected information. This work will primarily be conducted in the Emergency Department but may also include inpatient floors. Works only under the supervision of a registered pharmacist
