Job summary

Grane Rx is seeking a Pharmacy Services Liaison for the PACE program. The role requires on-site work at the PACE center with some remote work from home, full-time, and close collaboration with center staff, nurses, providers, and Grane Rx teams.

The position emphasizes resolving inquiries, managing pharmacy operations, training staff, and maintaining high patient-centered service standards to ensure continuity of care within PACE centers.

Qualifications

• Licensed California pharmacy technician preferred.
• Experience in long term care or PACE pharmacy preferred.
• High School Diploma or equivalent.

Responsibilities

• On-site attendance at the PACE center; some work from home as needed.
• Assist with daily pharmacy needs and resolve quality-related issues.
• Coordinate with delivery management and couriers for daily deliveries.
• Train center staff and refresh trainings on pharmacy protocols.
• Solicit feedback to improve standard processes and escalate issues.
• Process refill and tracking requests from clinic staff and participants.
• Communicate with outside providers to support continuity of care.
