Job summary

Penn Medicine in Philadelphia is seeking a Pharmacy Technician Care Coordinator to support medication access and adherence under pharmacist supervision. You will coordinate communication with nurses, physicians, and the medical team to assist patients and ensure timely prescription fulfillment.

The role requires CPhT certification and PA Registered Pharmacy Technician status, with 1+ years of pharmacy experience; 2+ years preferred.

Qualifications

• Pharmacy Technician certification (CPhT) required.
• Registered Pharmacy Technician (PA State Board) required.
• 1+ years Pharmacy experience as a certified pharmacy technician in a retail/outpatient payor environment required.
• 2+ years Pharmacy experience as a certified pharmacy technician in a retail/outpatient payor environment preferred.

Responsibilities

• Communicate and document appropriately and effectively with supervisors, physicians, nurse practitioners, unit based pharmacists, social workers, nurse managers, peers, patients/customers and outside agencies
• Collaboration with internal team members to gather appropriate paperwork, prescription, billing information, and/or prescription(s)
• Coordination of medication access and delivery for outpatient use
• Develop and maintain understanding of medication names and drug therapies especially for assigned service lines
• Provides appropriate follow-up relating to patients, clinical practices, and family for pharmaceutical needs and/or additional eligibility information to ensure on-going medication access
• Proficiently execute all accountabilities of Pharmacy Technician 1
