Job summary

TriStar Centennial Medical Center is seeking a Clinical Pharmacist to provide comprehensive pharmaceutical care and optimize drug therapies, ensuring safe, effective, and cost-efficient outcomes. You will work closely with physicians and nurses to resolve reactions, errors, and concerns.

The role requires Tennessee pharmacist licensure or eligibility, hospital pharmacy experience, and strong communication and computer skills, with ACLS/BLS certifications within required time frames.

Qualifications

• Current TN Pharmacist licensure or eligible for licensure.
• One-year hospital experience or equivalent.
• Hospital pharmacy residency preferred.
• Excellent communication and computer skills.
• ACLS within 60 months of employment start date.
• BLS within 60 months of employment start date.
• Doctorate Degree or Bachelor Degree.

Responsibilities

• Provide pharmaceutical care activities to ensure safe and effective drug therapy.
• Monitor and intervene in therapeutics to optimize outcomes cost-effectively.
• Fulfils department responsibilities within the hospital's mission and values.
• Collaborate with physicians and nursing staff regarding patient reactions, errors, and complaints.
• Determine and consult on potential drug interactions.
• Assist with quality improvement initiatives, inventory and ordering of pharmaceuticals.
• Monitor, record and communicate using computerized documentation systems.
• Apply age-specific knowledge in performance of duties and responsibilities.

Job Summary and Qualifications

The Clinical Pharmacistis responsible forproviding pharmaceutical care activities to ensure safe and effective drug therapy through the provision of clinical pharmacy services. The Clinical Pharmacist delivers pharmaceutical care services by monitoring and intervening in therapeutics to provideoptimumoutcomes in acost-effectivemanner. The Clinical Pharmacist fulfills departmental responsibilities within the long-range plan, mission,visionand values of the hospital.
