Job summary

PromptCare Companies is seeking a Home Infusion Pharmacist to join our per diem team in Cedar Rapids, IA. You will review charts, lab results, and physician orders for patients receiving complex infusion therapy, ensuring safety and therapeutic appropriateness.

Work schedule is Mon-Fri 8:00am-5:30pm CST as scheduled, with patient contact primarily by phone and occasional in-person visits in the infusion suite. Prior hospital and home infusion experience is valued.

Qualifications

• B.S. or Doctor of Pharmacy degree required or licensed pharmacist if foreign graduate
• Active registered license in Kansas; additional states may be required
• 1 year hospital and home infusion therapy experience preferred
• Two years of recent clinical pharmacy experience preferred
• Thorough knowledge of infusion therapies preferred
• Recent inventory management experience preferred
• Pharmacist CPR+ software knowledge preferred

Responsibilities

• Participate in patient evaluation and chart review for acceptance and supply management
• Collect, interpret, and evaluate physician medication orders for therapeutic appropriateness, cost effectiveness, accuracy, safety and completeness
• Monitor patient progress and communicate clinical and technical information with the physician and healthcare team
• Directly supervise pharmacy technicians and support personnel in compounding, packaging, and labeling of CSP
• Assist in training and orientation of new employees, performance improvement activities and educational programs
• Additional licensure may be required in multi-state service areas
• Go into IV room and mix medications if needed
• Other duties assigned

You will work closely with physicians, nurses, and other healthcare professionals to ensure that patients receive the highest quality of care. Requirements

• B.S. or Doctor of Pharmacy degree is required or Registered pharmacist license if foreign graduate
• Active registered pharmacy license in the state of Kansas and additional states as required without restrictions
• 1 year of hospital and home infusion therapy experience preferred
• Two years of recent clinical pharmacy experience preferred
• Thorough knowledge of infusion therapies preferred
• Recent inventory management experience preferred
• Accuracy in mathematical calculations including pharmacokinetic dosing preferred
• Knowledge of CPR+ software preferred

Job Responsibilities, included but not limited to:

• Additional licensure may be required in multi-state service areas
