Job summary

PromptCare in New Providence, NJ seeks a skilled Home Infusion Pharmacist to review charts and labs for patients receiving specialized infusion therapy, collaborating with physicians, nurses, and other healthcare professionals to ensure high-quality care.

Qualifications

• Bachelor of Science or Doctor of Pharmacy required; registered pharmacist license if foreign graduate.
• Active pharmacist license; Kansas and other required states without restrictions.
• 1 year hospital and home infusion experience preferred.
• Two years of recent clinical pharmacy experience preferred.
• Thorough knowledge of infusion therapies preferred.
• Inventory management experience preferred.
• Accuracy in pharmacokinetic dosing calculations preferred.
• Knowledge of CPR+ software preferred.

Responsibilities

• Participate in patient evaluation and chart review for acceptance, product selection, device selection and supply management.
• Collect, interpret, and evaluate physician medication orders for therapeutic appropriateness, cost effectiveness, accuracy, safety and completeness.
• Monitor patient progress and communicate with physician and healthcare team; most contact by phone.
• Directly supervise pharmacy technicians and support personnel in compounding, packaging, and labeling of compounded sterile product.
• Assist in training and orientation of new employees, performance improvement activities and educational programs.
• Additional licensure may be required in multi-state service areas.
• Go into IV room and mix medications if needed.
• Other duties assigned.

We are seeking a highly skilled and motivated Home Infusion Pharmacist to join our team. As a Home Infusion Pharmacist, you will be responsible for reviewing charts and lab results for an assigned caseload of patients receiving specialized, complex infusion therapy. You will work closely with physicians, nurses, and other healthcare professionals to ensure that patients receive the highest quality of care. Requirements

• B.S. or Doctor of Pharmacy degree is required or Registered pharmacist license if foreign graduate
• Active registered pharmacy license in the state of Kansas and additional states as required without restrictions
• 1 year of hospital and home infusion therapy experience preferred
• Two years of recent clinical pharmacy experience preferred
• Thorough knowledge of infusion therapies preferred
• Recent inventory management experience preferred
• Accuracy in mathematical calculations including pharmacokinetic dosing preferred
• Knowledge of CPR+ software preferred

Job Responsibilities

• Additional licensure may be required in multi-state service areas
• Go in to IV room and mix medications if needed
