Job summary

PromptCare, Inc. seeks a skilled Home Infusion Pharmacist to review charts and lab results for patients on specialized infusion therapy, collaborating with physicians, nurses, and other healthcare professionals to ensure high-quality care in Cedar Rapids, IA.

This per diem role follows a Mon-Fri 8:00am-5:30pm CST schedule; requires a B.S. or PharmD and an active license. Preference for hospital and home infusion experience, and knowledge of infusion therapies, is noted.

Qualifications

• B.S. or Doctor of Pharmacy degree required or registered pharmacist license if foreign graduate.
• Active licensed pharmacist in the state of IA and additional states as required.
• 1 year hospital and home infusion therapy experience preferred.
• Two years of recent clinical pharmacy experience preferred.
• Thorough knowledge of infusion therapies preferred.
• Recent inventory management experience preferred.
• Accurate pharmacokinetic dosing calculations preferred.

Responsibilities

• Participate in patient evaluation and chart review for acceptance, product selection, device selection and supply management.
• Collect, interpret, and evaluate physician medication orders for therapeutic appropriateness, cost effectiveness, accuracy, safety and completeness.
• Monitor patient progress and communicate clinical and technical information with the physician and healthcare team, primarily by phone with occasional in-person visits.
• Directly supervise pharmacy technicians and support personnel in compounding, packaging, and labeling of compounded sterile product.
• Assist in training and orientation of new employees and participate in performance improvement activities.
• Additional licensure may be required in multi-state service areas.
• Go into IV room and mix medications if needed.
• Other duties assigned.

We are seeking a highly skilled and motivated Home Infusion Pharmacist to join our team. As a Home Infusion Pharmacist, you will be responsible for reviewing charts and lab results for an assigned caseload of patients receiving specialized, complex infusion therapy. You will work closely with physicians, nurses, and other healthcare professionals to ensure that patients receive the highest quality of care. Requirements

• B.S. or Doctor of Pharmacy degree is required or Registered pharmacist license if foreign graduate
• Active registered pharmacy license in the state of Kansas and additional states as required without restrictions
• 1 year of hospital and home infusion therapy experience preferred
• Two years of recent clinical pharmacy experience preferred
• Thorough knowledge of infusion therapies preferred
• Recent inventory management experience preferred
• Accuracy in mathematical calculations including pharmacokinetic dosing preferred
• Knowledge of CPR+ software preferred

Job Responsibilities, included but not limited to:

• Additional licensure may be required in multi-state service areas
