Job summary

PromptCare, Inc. in New Providence, NJ is seeking a Home Infusion Pharmacist to review charts and lab results for a caseload of patients receiving specialized infusion therapy, collaborating with physicians and nurses to ensure high-quality care.

This full-time role (40 hours/week, Mon-Fri 8:00am-5:30pm CST) requires a PharmD or BS in Pharmacy and an active state license, with infusion therapy experience preferred.

Qualifications

  • Pharmacy degree or license is required.
  • Active pharmacist license with state registration.
  • 1 year hospital and home infusion experience preferred.
  • 2 years clinical pharmacy experience preferred.
  • Knowledge of infusion therapies preferred.
  • Inventory management experience preferred.
  • Accuracy in pharmacokinetic dosing calculations preferred.
  • Knowledge of CPR+ software preferred.

Responsibilities

  • Participate in patient evaluation and chart review for acceptance, product and device selection.
  • Collect, interpret, and evaluate orders for therapeutic appropriateness, cost effectiveness, safety and completeness.
  • Monitor patient progress and communicate with physicians and healthcare team, primarily by phone.
  • Directly supervise pharmacy technicians in compounding, packaging, and labeling.
  • Assist in training, orientation, and performance improvement activities.
  • Additional licensure may be required in multi-state service areas.
  • Go into IV room and mix medications if needed.
  • Other duties assigned.

Description

We are seeking a highly skilled and motivated Home Infusion Pharmacist to join our team. As a Home Infusion Pharmacist, you will be responsible for reviewing charts and lab results for an assigned caseload of patients receiving specialized, complex infusion therapy. You will work closely with physicians, nurses, and other healthcare professionals to ensure that patients receive the highest quality of care. Requirements
  • B.S. or Doctor of Pharmacy degree is required or Registered pharmacist license if foreign graduate
  • Active registered pharmacy license in the state of Kansas and additional states as required without restrictions
  • 1 year of hospital and home infusion therapy experience preferred
  • Two years of recent clinical pharmacy experience preferred
  • Thorough knowledge of infusion therapies preferred
  • Recent inventory management experience preferred
  • Accuracy in mathematical calculations including pharmacokinetic dosing preferred
  • Knowledge of CPR+ software preferred
Job Responsibilities, included but not limited to:
  • Participating in patient evaluation and chart review for acceptance, product selection, device selection and supply management
  • Collect, interpret, and evaluate all physician medication orders for therapeutic appropriateness, cost effectiveness, accuracy, safety and completeness
  • Monitor the patient’s progress and assessment results to communicate clinical and technical information with the physician and other members of healthcare team. Most patient contact will be on the phone, and, on occasion, you will meet them in our infusion suite
  • Directly supervise pharmacy technicians and support personnel in the appropriate compounding, packaging, and labeling of compounded sterile product
  • Assist in training and orientation of new employees, performance improvement activities and educational programs
  • Additional licensure may be required in multi-state service areas
  • Go in to IV room and mix medications if needed
  • Other duties assigned
