Job summary

Life Tree Pharmacy, a Merakey subsidiary, seeks a licensed pharmacist to join its LTC-focused team in Coraopolis, PA. You will compound, dispense, review prescriptions and advise on drug interactions while ensuring compliance and safety.

The role requires a current pharmacist license and 2+ years of related experience; LTC/ECM experience is preferred. Schedule is Mon-Fri 8:30–5:00 with benefits and growth opportunities.

Qualifications

• Current Pharmacist License in good standing.
• Two+ years related experience and/or training.
• Framework LTC experience preferred.
• Framework ECM experience preferred.
• 1 year of experience in LTC pharmacy preferred.

Responsibilities

• Reviews prescriptions issued by Physician or other authorized prescriber to assure accuracy and determine formulas.
• Compounds medications using standard formulas and processes.
• Directs pharmacy workers engaged in mixing, packaging, and labeling pharmaceuticals.
• Answers questions on drug interactions, side effects, dosage and storage.
• Maintains procedures for quality assurance, security of controlled substances, and disposal of hazardous waste drugs.
• Enters data such as patient name, prescribed medication and cost into the system.
• Instructs interns and other medical personnel on pharmacy matters.
• Maintains perpetual inventory.

Life Tree Pharmacy is a for profit subsidiary of Merakey and works with a wide range of facilities and programs to support individuals in LTC as well as the Behavioral Health and Intellectual and Developmental Disability communities.

Position Details

Compounds and dispenses prescribed medications, drugs, and other pharmaceuticals for patient care by performing the following duties:

• Reviews prescriptions issued by Physician, or other authorized prescriber to assure accuracy and determine formulas and ingredients needed.


• Compounds medications, using standard formulas and processes such as weighing, measuring and mixing ingredients.


• Directs pharmacy workers engaged in mixing, packaging, and labeling pharmaceuticals.


• Answers questions and provides information on drug interactions, side effects, dosage and storage of pharmaceuticals.


• Maintains established procedures concerning quality assurance, security of controlled substances, and disposal of hazardous waste drugs.


• Enters data such as patient name, prescribed medication and cost, to maintain pharmacy files, charge system, and inventory.


• Instructs interns and other medical personnel on matters pertaining to pharmacy.


• Employs perpetual inventory.

The successful candidate will have the following:
