Job summary

PharMerica in Largo, FL is hiring a full-time Pharmacist to join our closed‑door, non-retail pharmacy team serving long‑term care and senior living clients.

You will evaluate orders, provide clinical recommendations, maintain patient profiles, and collaborate with physicians and nurses to optimize therapy while ensuring compliance with state and federal regulations. This role offers scheduling stability and comprehensive benefits.

Qualifications

  • Bachelor’s Degree in Pharmacy, PharmD may be preferred.
  • One year experience in long-term care desired.
  • Current and valid pharmacist license in the state of practice.
  • Knowledge of drug therapy guidelines, medication safety practices, and regulatory standards (CMS, DEA, FDA).
  • Proficiency in electronic medical records and pharmacy information systems.
  • Strong clinical judgement and effective written and verbal communication.

Responsibilities

  • Evaluate physician medication orders for drug appropriateness, correct dosage, potential interactions, and proper administration.
  • Provide clinical recommendations to physicians and healthcare staff regarding therapy changes and optimizations.
  • Maintain up-to-date patient/resident profiles including medical history, labs, and current medications.
  • Ensure lab tests for high-risk medications are ordered, reviewed, and communicated to the healthcare team.
  • Collaborate with physicians, nurses, and other healthcare professionals to provide clinical and technical support.
  • Document communications and interventions in the patient’s medical record, including interactions with families.
  • Implement and enforce policies for safe handling, dispensing, and oversight of medications.
  • Ensure compliance with state and federal pharmacy regulations and reporting of errors.

Schedule:
  • Monday, Wednesday, Thursday
  • Saturday, Sunday, Monday, Tuesday, and Thursday (every other weekend rotation)
Shift Details:
  • 8- or 10-hour shifts (8 hours will be 32 hours/week and 10 hours will be 40 hours/week)
  • Start time: 8:00 AM (weekend will be at 9AM (10 hours shift) or 10:30 AM (8 hours shift))

As a Pharmacist you will:
  • Evaluate physician medication orders for drug appropriateness, correct dosage, potential drug interactions, and proper route of administration.
  • Provide clinical recommendations to physicians and healthcare staff regarding medication therapy changes and optimizations.
  • Maintain up-to-date patient/resident profiles, including medical history, lab results, and current medications (prescription, OTC, and home remedies).
  • Ensure laboratory tests required for monitoring high-risk medications are appropriately ordered, reviewed, and communicated to the healthcare team.
  • Collaborate with physicians, nurses, and other healthcare professionals to provide clinical and technical support.
  • Document all communications and clinical interventions in the patient’s medical record, including interactions with families and caregivers.
  • Implement and enforce policies and procedures for the safe handling, dispensing, and clinical oversight/management of medications.
  • Ensure compliance with all state and federal regulatory requirements related to pharmacy practice and medication management.
  • Report medication-related errors and adverse drug events to the Pharmacy Manager as required.
  • Educate and support clinical team and healthcare agency staff on safe and effective medication handling procedures.
  • Perform other pharmacy-related tasks and responsibilities as assigned.

  • Position will be posted for a minimum of 7 business day
