Job summary

Yale NewHaven Health is seeking a hospital pharmacist to join our acute care team in Connecticut. The role requires knowledge of pharmacology, therapeutics, and strong communication to support clinicians and patients.

Two years of acute care hospital experience is preferred, with residency in hospital pharmacy desirable but not required. The pharmacist will participate in drug distribution, monitoring plans, and education, ensuring safety and quality in medication use across the department and

Responsibilities

• Resolves drug distribution problems and ensures accurate, timely dose delivery for patients.
• Interprets clinical and pharmacokinetic data to plan medication monitoring.
• Documents problem detection, findings, recommendations and follow-up.
• Provides drug information to nurses, MDs, and other health care professionals.
• Participates in and contributes to educational forums within the department.
• Documents medication use variances (MUVs) and adverse drug reactions (ADRs).
• Demonstrates principles of a High Reliability Organization and patient safety.

Responsibilities

• 1.Resolves drug distribution problems assuring accurate and timely dose delivery to all patients
  • 1.1Responds to telephone requests for medication in appropriate fashion accommodating callers request or routing call to appropriate person.
• 2.Interprets Clinical and Pharmacokinetic Lab Data in order to formulate medication monitoring plans.
  • 2.1Demonstrates knowledge of basic pharmacokinetic principles.
• 3.Problem Identification & Resolution
  • 3.1Communicates and documents problem detection, relevant findings, recommendations, and necessary follow-up.
• 4.Provides drug information to nurses, MDs, and other health care professionals
  • 4.1Serves as primary source of drug information to physicians, nurses, and other health care professionals
• 5.Educational Activities
  • 5.1Participates in and contributes to educational forums within the department (written or verbal).

Qualifications

EDUCATION

Graduation from an accredited College of Pharmacy with a B.S. or Pharm.D. degree. Completion of a residency in hospital pharmacy from a program accredited by the American Society of Hospital Pharmacists is desirable but not required.

EXPERIENCE

Two years experience in an acute care hospital pharmacy is preferred for this position.

LICENSURE

Licensed to practice pharmacy in Connecticut (or eligible for reciprocation)

SPECIAL SKILLS

This individual should demonstrate knowledge of pharmacology and therapeutics, and understand pharmacy systems of drug distribution, and formulary management. Excellent communication skills and the ability to work with others are required.
